With any programmable mouse you first figure out which function will be assigned to specific keys in the game itself. Then you use the program for assigning the mouse buttons to coincide. Otherwise the scroll button will scroll through the same function assigned by default in the game itself.

Press space or enter to use or activate item or climb ladder=second button. If the scroll wheel also sees a button you can assign that to jump to use item as well. In XP the MS mouse used here saw the space bar option for the second button while Vista sees the enter key. For both versions guess what? The enter key had to be assigned.

Right click is good for alternate weapon or reloading with the next key for double clicking. You have to decide which function you want it to perform in a game and then assign each using the MS part of it.
