Looks good, you might want to run a reg cleaner to clean up any leftovers there.
For the file that won't go, open Hijackthis and select "Misc Tools". Go to "Delete a file on reboot" and browse to this file
C:\Program Files\Visual Networks\Visual IP InSight\SBC\
IPClient.exe
and hit the "open" button, restart the machine.
I also noticed a few entries from Mcafee, when you seem to be running Norton. If you no longer have Mcafee, fix all the entries in Hijackthis, then go to the Program Files folder and delete any instance of Mcafee.
With all the junk on this machine, I think
Ccleaner might be a good idea. Uncheck "cookies" in the IE and Firefox tabs, it can be set to run at bootup under "options". Let us know how things are running now.
