not to sound like im moaning or anything, i tried to fill it out, but the questionarre is not written well
a) i use all of the q1 (same for q7)
b) for q6 i dont agree with any option available so i used the other box - however this prevents the form being submitted
c) The is a bloody pop up advert, never a good thing

oh and company size options is quite limited, 500 is not that big, my firm emplyees over 10k of people world wide,
