Default Weird HDD problem

Recently I have been encountering a problem with the HDD. Every time I defrag the C: drive (not sure about D, the mouse cursor starts to lag, the music sounds choppy, and the HDD activity led shows intensive activity.
When it's idle, the led looks kind of semi-on, so it starts and stops very quickly.

If I don't turn off the computer and restart it, it simply freezes.

What could be wrong with it? It's not the first time, it was happening a long time ago randomly, but it stopped, and now started again.
