Default smart failure predicted on hard disk 4

Had this message come up on my laptop "smart failure predicted on hard disk 4", all my data is saved and I take it the hard drive is goosed. If so is it just a case of replacing the hard drive and reinstalling windows vista with the original disc and product code, many thanks in anticipation.

Pretty much. You can replace the drive, reinstall window and restore your data back up and it should be good to go.
