Default Computer won't read SD card

Got a new phone. Have taken some pictures & videos with it. Put in a micro SD card and formatted the card in the phone. Transferred the pics & videos to the card. Took it out of the phone & put in the 'standard' sized SD adaptor & plugged into the PC. The PC doesn't do anything to indicate it sees the card. I go to MY COMPUTER and the 'I' drive does show the card being there. Double click the "I" drive. Takes forever for the hourglass to go away & I get a popup that says the card is NOT formatted & do I want to format now. Well, okay, YES go ahead & format now. After about 15 seconds I get another popup that says the card cannot be formatted. There is a little 'lock' switch on the adaptor. I have tried this in both the locked & unlocked positions.

Any help???
