Question Psu rma

My psu recently hit the dust. It just suddenly exploded with smoke and everything. I sent an email to the company and they said to fill out an RMA form from their site and fax it back to them. This is my first RMA so I'm not quite sure exactly what to do.

Do I just fax it to them or what? Dont I have to ship them the defective psu too?
