Identify display adapter and reinstalling Windows NT server 4.0

I would like to check with all of you that: -

a) what is the software that I could use to identify my exact buildin display adapter which is running under Windows NT server 4.0?

b) Besides that, what are the steps that I could follow in order to let me reinstall my Windows N. T 4.0 Server?
