Default Smart

SMART monitoring is there to inform users about the state of the HDD and to inform the user of any problems possibly approaching. It seems that your HDD is damaged and may have DIED.

Default well

From what I have read, yes your HDD is either dead or very close to its death. However, if you have the means to backup, turn of SMART monitoring in the BIOS and attempt to boot up. If that works, backup for your life. I would recommend you get a new HDD just to be able to backup in case you manage to keep it alive longer.
