Default meaning of message

Hi,
I was wondering if someone could please tell me what the following message means in the most basic, general terms. Don't worry about the numbers; just tell me what the system is trying to do and why it isn't happening. Then, if you can, what the numbers mean in that context.

Application Error
Instruction at 00427595 referenced memory at 0000004e. The memory could not be written.

Alternatively, the same message, different numbers, change "written" to "read".

I'm also interested in what 0000004e refers to.

EDIT: (If this helps...I'm running Windows XP with SP2; the program in question is an old game (Doom 95)).

Thank you so much,
Jason
