I see you have an older trojan downloader. Besides a look over of the log you may want to give a few removers a try. http://www.securemost.com/articles/rm_vcodec.htm

But the alarm isn't so much about that type of pest but on the Realtek "bug" found that will need to be removed manually. "C:\WINDOWS\ALCXMNTR.EXE" is today's winner there! AVG has apparently kept a lot of other crap off of your system. Ignore the usual "(file missing)" seen on the leftover registry value here. "O20 - Winlogon Notify: WRNotifier - WRLogonNTF.dll (file missing)"

I am unable to delete C:\WINDOWS\ALCXMNTR.EXE it tells me that my access is denied. The fix page that you linked also was not helpful. The spyware doctor found 1118 infections, unfortunately, I have to buy it to remove them. So I am back to square 1

The so called 1,118 infectiions non sense is just that. "NONSENSE"! It's a selling gimic to draw you in to buy the retail version while nothing is actually there. There's a number of sharewares with the same crap telling you "buy the full version to see these removed".

The log was clean except for a familiar annoyance namely the little bug placed for collecting information on you! To remove the file found you first have to disable it if seen in the processes in the task manager or simply boot up in safe mode.
