Default (C:) + (D:) = two hard drives?

I'm been using an old computer for a while: the hard drive should be about 15GB, but it's split into two different disks (C: and D. I don't know much about computers, but does this mean that the computer holds two different hard disks, or is it just one that has been split in two in some way? Thank you.
