Default PC Phone Logging

A client has approached me with a problem that is outside my own experience, so I figured I would post here and see if anyone can point me in the right direction.

This is the clients problem in his own words:

I'm trying to set up a system, so that a modem in one of my computers can monitor the telephone, and either display a message or play a sound or both when it detects that the phone is ringing. I also need it to log when rings happen, and I need to know how to get this going on both Windows XP and Ubuntu Linux (ideally, OSX as well, but that's not critical).

I would recommend voicemail or answering machine software. They usually can do this. Just google for it, as I haven't done this sort of thing for a long time, and the software that I did use is not compatible with recent versions of Windows.
