Any software that supports form creation and form filling function?

Hi all,

I would like to know if there is any software that supports form creation and form filling function. My meaning of form filling is that, after creating the form, the software will read a data source file, i.e. MDB, and automatically fill in the form by matching the field names in the form and the fields in the data source file.
