This sounds exactly like the problem i had with my 6950 that would just black the screen as soon as i put it under any real stress....sounds very much like failed card.
Op have you been able to try another graphics card in your system?
Yeah I have had another graphics card in it but I took it out and replaced it with the one I have because the older one couldn't play anything.
