What on Earth is this? What adapter is needed

One of my work colleagues has sent me a picture of there "graphics card" and asked me to take them an adapter that will work for it.

If anyone has any ideas what this is a picture off, it would be very much appreciated! I have convinced myself that it is not a gfx card, but i may be wrong!

What is the model number on the card? Looks somewhat like a DVI port but not exactly.
