If you don't press a key when prompted, the computer will continue to boot from the hard drive as normal. If you haven't tried that yet, I suggest you give that a shot.

In regards to upgrading 64bit vista to 64bit windows 7? what happens to all the programs and whatever else? Is it a worthwhile process or would one be better off doing a fresh install?

A fresh install is a better option if you can stomach the software re-install process. On the other hand, a direct upgrade should work just fine. Just to be safe, you may want to run the Windows 7 upgrade adviser. That will tell you if there are any incompatible programs or software that will need to be re-installed after the upgrade anyway.
