2 file extensions?

What would cause a file name to have 2 file extensions?

like: winner.txt.txt?

How would I solve it and how would I prevent it happening again?

Any ideas would be greatly appreciated as it's for coursework, that doesn't seem to be covered in my module.

EDIT: I've actually been renaming files (hypothetically) so it must be something I've done?!?
 
The first .txt is in the name and the second is the extension, just rename the file and do not put .txt at the end , it will do that it self
 
Open my computer --> Tools --> Folder Options --> View --.> and then uncheck the "Hide the extensions for known file types"
It could prevent viruses to be got on ur PC, SInce u will know the right extension for the file
