A 360 (triple 120) would be fine. I have never seen a case that supports a quad, only external setups.

yes.

You can arrange them any way you want to. you can use 2 triples, 2 doubles, a triple and a double, single and triple, single and double three singles. Really it will wont make a large difference.

You only need a 240 for that. I run GTX580 SLI OC to the hilt on a 240, ill doubt you will need more surface area. Just make sure the flow rate is high. Also don't get caught up in the order and positioning of things, it makes no difference as it all ends up in equilibrium.
