Best card for my aging desktop

Simple question for a PC guru: What would be the best video card upgrade for my system (try not to laugh too much):

AMD Athlon™ 64 X2 5000+, 2048 MB DDR2, 667 MHz, unsure of the mobo (yes, I'm a lazy mofo), it's a Gateway GM5472

Looking to buy a PCI Express ×16 upgrade, but why buy a $200 card if the system will bottleneck at a $100 card?

Currently using my 27" LCD tv with the NVIDIA GeForce® 6150SE integrated video without missing a beat, but looking to play some slightly modern games (all genres) at medium settings.

Naturally, I'm willing to upgrade the PS if it's cost effective, but also willing to ditch the whole system and go for something new.

Yes, I'm sure this question is beneath many of you, but I respectfully ask your advice.
