Disk Defragment

I tried defragmenting my disc several times , but everytime a lot or atleast few files are left fragmented with an error that few files could not be defragmented blah blah blah , and whenever i see the report .. i've noticed that few of the files are the same .. whats thats happening?
 
It's most likely because the files are in use. Before running Disk Defragmentor, close out of all running programs that you dont need.
