dying hard drive help

Hey there guys I was hoping I could get some help to identify if my hard drive is bad/dying.

For a while now my computer has been randomly freezing and I thought it was a software/os problem so I have formatted and reinstalled windows a few times but that doesn't seem to fix the problem. Not knowing much about hard drives I never thought that could be the problem but I'm thinking it is now.

I installed active hard disk monitor and here is what it says.

Don't really know what to make of that info!
 
What type of your hard drive is?
Maybe it doesn't support with your OS. Reinstall OS can't solve this problem.

So I searched and figured out I have a seagate hard drive.
I downloaded seatools (is that the right utility?).

It seems there are a series of tests I can run.
Any one or two in particular I should share with everyone?
 
You should be concerned with only 2 tests at the moment, short and long(or extended). Do the short test first and see what the results are and then do the long(extended test).
