Facebook adding friends without permission?

My sister is telling me that for about the third or fourth time, she's getting a notification that someone has accepted her friend request on Facebook. However, not only has she not sent any requests, she doesn't even know these people!

Has anyone heard of this happening, or know how to stop it?

Remove the friends she doesn't know and change her password. Someone may have access to her account.

I considered that, but nothing else weird has been happening that I know of. Still, might as well do that at least.
