Have anything else being plugged in along with a mouse? If you have another device going on you that could be pulling the bus down. A flaky supply with poor regulation can cause problems.

One suggestion would be to toss the old supply back in to see if the problems continue. If everything seems to be back to normal flaky supply. But if you still see problems look at other things like other usb devices like a printer or possibly the board itself? Bad ports from wear?

The method of isolation one item at a time should help to see where the problem is. Try using a PS/2 adapter for the mouse for the time being so that will function normally.
