How to create a Windows boot disk w/ cd rom support?

Would someone give me the instructions to creating a windows boot disk with cd rom support. I want it on a 3 inch floppy drive.

Need it to work for 95, 98 and xp. I realize I might have to make seperate disks. Please give instructions.
 
do you mean a 2.5" floppy?

You only need a boot disk for 95 and 98, Windows XP just uses the Install CD.

When your in windows 95/98, go to add/remove programs, and somewhere around there should be an option to create a boot disk.
 
Yes. I meant 2.5 disk

Yes. I meant 2.5 disk. I want it on the disk just for emergencies. I realize that the original install disk has it built in, but I need the 2.5 disk also.

Please assist
