Have you tryed Spy Sweeper? It's a 14 day free trial and it gets rid of more spyware than most free spyware removers. (According to PC World)

Also you can try ewido. It does a massive system scan and detects most spyware.

If you get Ad-Aware Pro, it comes with Ad-Watch that monitors and blocks spyware in realtime.

Thats good for spyware removal, but it doesnt block spyware/adware in realtime.
