Identify pc board

If I am not in the proper forum, please correct me, or move the post, and please accept my apology.

I have a small board on the front of my computer that has two usb ports and a 3.5 mm input mic jack and 3.5 mm speaker jack. The 3.5 mm input jack (green) does not work and I need to either replace the jack or the whole board. I cannot seem to identify the board and wondered if someone could help me. Below are numbers and attached photos of the board.

The number on the board are: E116921, and 3RAM63000902 rev: 1.2

Can you do a closed loop test by using a 3.5" broken out into leads? That'd tell you board or jack itself if you can't get circuit continuity.

What kind of cable are you trying to use between the headers of that board and the audio headers of the motherboard? A lot of the time there will be a dual AC97 and HD Audio one depending on the sound card side.

Hi Beers,

Let me shut down the computer, and remove the board and take a picture of the leads to the board.

I do know it is the jack that is at fault. I attempted to disassemble the jack, but of course ruined the jack. The jack appears to be "vertical" type.

Beers, it sure looks like the same jack. 4 pins lined up properly, a fifth up front.

I forgot to add that the jack senses whether the input is plugged in or not. Will that make any difference with this jack? If ok, great. I was looking all over for a place to buy one, need as little down time as possible. So thanks for a source as well.
