Looking for some help on how to create a database.

I am the owner of a local poker league in York, PA. I am looking for a way to incorporate the following into my website:
1. Players can sign in with a Name and password
2. Players can preregister for poker games by clicking on a preregistration link on my website
3. A list will be generated of the people that have prereigistered
4. Want the players to sign in with first name and last name and a password
5. Players be able to request a forgoten password
6. Players will be able to sign up on the website for my league

Any ideas on going about this?

I would search on microsoft.com They have some really good database templates that you can easily download free of charge.
