Turn a "scanned image" into a form?

Hello,

I have been asked by someone to turn what is a delivery note that is normally written on, into a "computerised" image in whcih the user can type on to.

I don't have access to the original document so I scanned the image onto my machine, but now I am not so sure what to do. I have tried using word to drape text boxes over the relevant bits but that didn't work to well due to the size of the text boxes etc and seemed quite fidderly.

Is there an "easier" way that someone may suggest turning a delivery note image into something that can be typed on?

The normal route would be the "someone" receiving the delivery note in a PDF format and then running something like Acrobat.
A crap workaround if you are only able to work with a scanned image would be to use OCR (Optical Character Recognition). Open up Microsoft Document Imaging or any other OCR program and it will extract the text into a Word document that you could then edit. It can extract images as well but not quite end up in their right position, these would probably need repositioning.
 
Thank you for your suggestion. Not sure OCR would be of any use here as I want the layout more than the text.

Worse come I will just have to create the crappy thing from scratch, I was just trying to avoid that
