Weird pop-up in Gmail - anyone else getting this?

For the last week or so, every time I go to my Gmail a pop-up box appears in the lower right corner, saying "In the last year, have your Gmail or YouTube accounts been hacked?" with a yes and no option. I always exit out of it, but I find it weird that I'm being asked this...
 
Do you have any weird add-ins installed on your browser? Do you have any type of ad-blocker enabled? I haven't seen or heard of this before.

I keep getting stuff like this on other sites, like YouTube, and occasionaly just at random wherever.
