What causes this artifact?

Blue/yellow 1/4" x 1/4" squares popping up across screen?

I have a powercolor 4830 and have had this problem for as long as I've had it i believe. When was using XP it came up after the 2nd or 3rd time after putting in hibernate and powering back up. now that I'm using Win 7, as soon as I touch either of the gpu clocks, up or down, I start seeing this issue. Is this just a bad gpu? I've been able to play games in the past, haven't tried since my recent upgrade. I've tried HDMI and VGA to my monitor and receive the same results.

have you ever done a full driver delete and update? use the normal instillation method, then reboot and use driversweeper to clean all registry entries out and any left over files, reboot, then install the latest drivers
 
Its overheating VRAM or an underpowered PSU, since you have the CS600, i would suggest its the heat issue.
