What's wrong with this video card?

I baught an MSI Ti4600 AGP card from a friend he says this MSI video card has been installed and did work properly upon initial installation. 6 months after installation, the card would heat up, causing the screen to pixelate. The card does not work properly. The card will install into a machine and work until the chip gets too hot so this may be a fan or heat sink issue. he says hes only guessing at the possible defect. Do you guys know what the problem is just incase?

I want to buy a new fan and heatsink, but i dont know witch one, Can a normal gf4 mx-440 from my old one work on this gf4 ti4600? Do i need to buy a heatsink/fan for gf4 ti's only, or gf4ti4600s only? and also, if you put a really powerful fan/heatsink on a weak video card would it work, for example, putting a 7800 GTX fan/heatsink on a 6600 GT, or on a Geforce 4?
 
first of all, clean the heatsink of the card again , my guess is that dust is blocking the normal exhaust way of the heatsink
