Always loosing connection

I'm constantly loosing connection, which is really annoying especially with online gaming.

Is there a piece of software that can monitor my connection in real-time, and produce like a graph or something to see when it's happening and how often?
 
When you get disconnected, what do you do to re-connect it? Do you restart your router?
How often do you get disconnected?

There is a few software out there that can do that but that really isn't necessary right now. I think I may know what the problem is.
 
When it disconnects, I pull the power leads from the router and modem, then put them back in. I've always done both but it might work with just one or the other.
I'd say it cuts out once or twice a day, but doesn't seem to follow a pattern.
