Artifacts, freezes and screen flashes

I feel stupid, but ever since I bought my ATI HD4870 I have had problems with artifacts and screen flashes. I've dealt with this problem for years by reducing settings in every program and ignoring the flashes. But now, when games started freezing a few months ago, it got really annoying. Therefore, I decided to finally face my problems and find a solution!

I know for sure that my card can do a lot more than what I ask of it. I know that there are a lot of articles about the 4870, and also about artifacts. Yet, I have the feeling that every situation is different and I don't know where to begin in my situation. First I want to fix the problems I am already facing when completely underclocked, and then I want to OC it to max stock values.

I included a report with the hardware info Everest gave me (had to zip it, because it exceeded the max for a .txt). Hopefully it is useful. If you need more info, or pictures/movie material of what kind of artifacts I am talking about, I'm happy to provide it.

If you guys are able to help me, I would be very grateful!

What make and model of power supply are you using? If the power supply unit is a good one then we can only assume that you have a bad card. Do you get any artifacting or issues before windows boots up?
 
Corsair TX 650 Watt, which should be more than enough right?

I also thought I had a bad card, that's why I immediately RMA'd the thing and I got a new one. The problems didn't go away. So I thought the problem wasn't with the videocard.
 
Was that Everest reading done at idle? If so the GPU is running kinds hot, its between upper 60c to low 70c. Plus you sure you have the refresh rate set right for the monitor?
 
There is only one option for the refresh rate and that is 60 Hz.

GPU was idle at the moment. I will clean my fans tomorrow and remove some dust to see if it makes any difference in temperature. Unfortunately, this will not solve any of the artifacts and flickering because when everything was brand new, it was already acting weird.

Well the card is running hot. Most 4870 I've had run alittle cooler then that under load. Plus your CPU is running at 51c. I would say just alittle load its heating up pretty much. Do you have good case air flow?
 
I have an exhaust fan which I don't use right now, would it be wise to put this under my GPU? Or would it make things worse?

I think the airflow is fine, but perhaps there's not enough hot air leaving the case, so maybe the fan could help?
