Blue Screen 5 minutes after login

Hi Computer Forum,

From middle school to junior year of high school I used a PC computer that my brother and I put together, replacing parts over the years when necessary. It worked without fail until it randomly started crashing while playing Dark Souls, Awesomenauts, and many other games. In the beginning, the crashes were rare and tolerable. After a few months they got more common, crashing during youtube videos as well. As college approached, I desperately tried to fix my computer, replacing the motherboard. Nothing changed. I'm currently using my brother's old computer at school, with my own computer at home. Over the school year I have tried multiple times during breaks to fix my own computer that is practically unusable. At this point, the computer will crash only a few minutes after login, making it hard to retrieve and important information.

TL;DR
My computer is basically unusable, because it crashes to blue screen right after logging in. Used to only occur while playing video games.

Here are a few things that might help:

1) I installed CoreTemp to monitor the computers heat in relation to the crashing - No relation, the heat never spiked or increased erratically
2) I replaced the mother board - No Change
3) I removed the graphics card and used it in my brothers computer - my brothers computer works fine.
4) I installed various different drivers for the graphics card - No change
5) Tried different combinations of my RAM, even replaced it eventually - No change
6) Reformatted countless times

Computer Specs: (Although I don't think these are all the specs, I don't know what I did wrong in retrieving the information. Maybe someone could give me a link to finding more detailed information)

Windows 7
Processor: AMD phenom(tm) II X3 720 Processor 2.80 GHz
RAM: (2x) ripjaws 8gb
Graphics Card: Radeon Sapphire
Power supply: x-power 585

Blue Screen Test: (I took this test back in December, it's still a mystery to me why the results are so dated....)

Any help would be appreciated, I have tried a few times on here before, but school has always separated me from the computer and actually being able to do anything. I'm home for the whole summer, so I will have plenty of time to fix this computer issue.

Did you replace all of your RAM?

What temps are you getting with CoreTemp for the CPU and GPU?

Your PSU is completely unsuited to the 4800 series graphics card and is very likely causing the issue.

I've been wondering if the power supply might be responsible, however, why didn't I have these crashes since the very start?

That's the PSU! I'll replace it ASAP! Is it possible I might need to replace anything else due to damage?

EDIT: Also, unless Okeydokey has a different recommendation, I'm just going to buy WeatherMan's recommendation.

Would it be a bad idea to replace the graphics card as well?

Weatherman's recommendation is fine.

I'd start with just the PSU and see if that fixes the problem.

Update: just reformatted, running on a single harddrive, lasting longer then 5 minutes, but this usually happens after a reformat

When it blue screens again, zip up the actual dump file and attach it to your reply. They will be located at c:\windows\minidump.

So my computer has been running fine since I reformatted. However, the only difference is that I booted from only ONE of my hard drives. Is it possible that the other one is corrupt? Anyways, I'll post an update if end up blue screening anyways, but thank you guys for your help!!
