Computer goes power save mode while gaming

Hello everyone, I'm new on this forum.

Basically my problem has always been there but there were times when it didn't happen at all and then it happened to me many times a day. So when I'm gaming, my screen goes black and says: Entering in power save mode. Or it says something about the resolution but it does the same thing. Then I can't do anything, ctrl alt delete or anything. So I just hold the power button and reboot it. I will post pictures soon of the errors. This problem seems to be back again and it didn't happen to me for like a year.

Specs: Win 7 Ultimate
6 GB RAM
Intel core 2 quad 2.4 GHz
64 bit
ATI Radeon HD 4800 series
Dell SP2309W ( digital ) Screen
Dell XPS 430
 
This is not power save mode, you're losing signal to the monitor.

Either one of the following:

Insufficient PSU 12V rail
Overheating graphics card
Overheating CPU
Overheating RAM

Try completely re-installing the graphics driver.
Then download MSI Afterburner and run it. Play a game for ~15 minutes and then exit the game and review the temperatures.
