Disconnection problem in XP

I keep getting an intermittent disconnection in XP. It makes the "remove hardware" sound, i.e. when you unplug something from a USB port. What steps do I need to take to determine if something is loose/conflicting, etc?

Could be anything from bad power supply, bad motherboard, bad driver, anything. What have you done so far to try fixing this problem?
 
I think I narrowed it down. I think it's my new cardreader. I was transferring files and it disconnected.
It was hard to narrow it down because I installed the card reader, PCI-USB2.0 card, Wireless network card, fan controller (which had me in a panic because I thought I may have been running my PSU too hot).

So how can I fix this? Uninstall and reinstall the drivers for the card reader?
 
Probably, the card reader or port that it is plugged into could even be bad. So, I'd try reinstalling the drivers first, then changing the port, and then seeing if you can get a replacement reader as a last resort...
