Freezing problems while gaming,..

I'm having this problem for mos. now. Randomly, while playing games my pc just freezes with the looping sound in the background then dead silence a few seconds after. I thought it was a psu problem so I did change my psu, but it still freezes randomly. I already reformatted my computer a couple of times now, reinstalled my video card driver from oldest to the lates version, still no luck at all. While searching the web, I came into this video card stability test, so tested my video card with it and this what came out: http://i255.photobucket.com/albums/hh133/Aurlan/freezing-1.jpg

I did the test several times already, and there are times that the test would run ok, there are times it fails, so can you guys explain the meaning of the result? My video card is a 7600GST, a 7600GS which is factory overclocked to perform as fast as a 7600GT. The card is still almost 6mos. old and I had never overclocked it. Please help, thanks.

51 celsius while idle, 60-64 while gaming,......could there be something wrong with the fan? like it is slowing down or something? everytime the freezing occurs I always check whether the gpu fan is working and it looks okay.
