GF9800GX2 switching off monitor

Hi guys. I've had the following problem for over a year now, and I'm really getting sick of it.

At seemingly random occasions, although mostly while I'm playing games, my screen will go black, and I'll have to cold reboot to get back into Windows. I figure there could be various reasons behind this, but I figure overheating to be the top candidate.

This is my rig:

Microsoft Windows Vista Ultimate 64 med SP1.
CPU: Intel(R) Core(TM)2 Quad CPU Q6700 @ 2.66GHz, 2660MHz
MB: ASUSTeK Computer INC., P5K Premium
Memory: 8gb ram
Videocard: ASUS Geforce 9800GX2 (1gb dual core)
HD: Samsung HD103UJ ATA (1TB)
Power supply: Not sure, but it says 700w max power on it.
Cabinet: CoolerMaster Centurion
Temperatures: CPU: 31C, Core 0-3: 39-46C idle.

I did download GPU-Z to check for signs of overheating, and it says the GPU is at 77 degrees celsius while only web-browser and skype is running, but the fan is still at 100% speed. Once I start a game, World of Warcraft for example, it almost instantly jumps to 85-88 degrees celsius, while the fan still at 100%.

So, guys, any idea?
