Help Me! Bad Textures!

Why do all my games look pixelated and grainy even at 1080p and max settings? Older games like San Andreas and HL2 are fine but newer ones have problem. Its especially bad in fallout 3 and witcher 2, the textures are not crisp at all. I'm getting good fps though. Things that are supposed to be smooth are sand like all grainy. When I move character it looks terrible. Please help! In crysis everything looks good except the foliage which is also pixelated even at 1080p, max sett, and 8xAA! Everyone is saying how good the witcher 2 looks but for me its cr@p at max settings, please help!

My Specs:

HD 6870
i5-2500
8GB Ram @1333mhz cas9
Samsung Spinpoint F3 1TB 7200rpm

I have latest driver too. Has my Radeon gone bad?

Does your monitor support 1080p and the resolution you're using? You say "even at 1080" like it's a low setting that isn't very demanding. Especially with max settings, it could be pushing it too far. I suggest trying just below max settings and see if it improves, or lower resolution. Both video cards and monitors alike have max supported resolutions.

My monitor natively supports 1080p and the card supports upto 1600p. I know its not a low resolution but it one this card is supposed to be able to handle.
 
Never mind all, I sorted it out. First stupid RadeonPro was causing LOD and texture problems. Even If exited the program the settings would stick and override everything else including in-game settings so I uninstalled it and reinstalled video drivers which it apparently corrupted. The second problem was catalyst AI, I turned that garbage off. It was causing LOD glitches left and right, things would look ugly unless I got within reaching distance. Playing Witcher 2 again and it looks GORGEOUS! Thanks for the replies though.

Actually I take that back it wasn't the catalyst AI. Getting rid of RadeonPro certainly helped though.

By bad textures do you mean all pixelized? Are you using AA? Are you using any software other than CCC?
