Mouse Problem

Hi,
While I'm playing sometimes my mouse, instead of staying with the game mouse just goes default and I have to return to the desktop and click on the game icon to continue playing...
Anyone can solve this for me?
 
could you be accidentally pressing something on the keyboard to make it do this? for example if you hit the windows key it can take you out of the game
 
My right mouse button stopped working. Needless to say I no longer can perform many movements and I’ve exhausted all my ideas on how to repair the problem. I can't find any “reset default settings” option anywhere, and it’s not my mouse.
How can I fix the problem?
