New GFX card needed I think

I think my GFX card is starting to go out on me. I did a Rebuild about 2 years ago and re-used my old SAPPHIRE 100251SR Radeon HD 4870 X2 2GB 512-bit (256-bit x 2) GDDR5 PCI Express 2.0 x16 which was bought about 5'ish years ago or so when they first came out.

I have been having problems with it lately, Freezing and crashing to the blue screen of death under a small to medium load of playing Everquest 2. I am almost 99% sure that it is the GFX card as I hear the Fan spool up quite often.

I would like some suggestions on replacing it if possible, Budget wise 300ish.
This is the rig it would be going in:
MSI 890GXM-G65 AM3 AMD 890GX SATA 6Gb/s USB 3.0 HDMI Micro ATX AMD Motherboard

AMD Phenom II X4 965 Black Edition Deneb 3.4GHz Socket AM3 125W Quad-Core Processor

CORSAIR XMS3 8GB (4 x 2GB) 240-Pin DDR3 SDRAM DDR3 1600 (PC3 12800)

850watt Power supply

Would Crossfire be feasible or needed? I do a decent amount of gaming.

300 bucks its either a GTX 560TI or a 6950. Unless you can get a good deal on a GTX 570 or 6970.

for the crossfire/sli part it depends on the brand of the psu but if it is a good 850w then it should be fine since even the 580 will run sli'd on a 750w. for the gpu, yeah, what strangle said, a 560ti or a 6950 would be great, although i did recently see some benches where the 460 sli'd was beasting and they don't cost much if someone has an opinion on that. and also just cuz you got 300ish doesn't mean that paying 300 for a gfx card that you could get for like 250 is worth it, generally there is very little difference for how much you're paying

With this upgrade will I see an increase in performance? I know things can be bottlenecking, I guess my question is how out dated is my old card?

your old card was fine but with the new ones you'l get the newer dx11 and opengl and other technologies that are more recent.
