physical memory dump problem

In the past 6 monthes every time i run a video game on my computer, after a while (about 15-20 min) the game is freaking out and that cause the computer to freeze and than the blue screen of the physical memory dump and restarts.
For example if i'm playing NBA 2k after about 15 min the sound of the broadcasters will stop and then the game will have no sound at all. When its half time or the end of the quarter the game will freeze after and after about 3 min it will show the physical memory problem for a sec and then it will restarts. Its not just on NBA 2k, this problem is on every game i tried to install on my PC (it doesn't matter if it is a heavy game like Crysis 2 or an older game like Warcraft 3).
I tried almost everything in mind. In the beginning i thought maybe the computer temperature is very high (maybe the video card or the motherboard temperature in high) while running heavy games but i check it and this is not the problem. I've also updated all the drivers, video drivers and DrictX and all these stuff.
I'm pretty desperate and need your help guys.

First of all, give me a list of your system specs.

1. make and model of pc if store bought
2. model number of motherboard if custom built
3. model of video card you are using
4. Model of Power supply
5. How much system memory is installed
6. What processor are you using

Then do the following and post the results.

Download BlueScreenView
No installation required.
Unzip downloaded file and double click on BlueScreenView.exe file to run the program.
When scanning is done, go Edit>Select All.
Go File>Save Selected Items, and save the report as BSOD.txt.
Open BSOD.txt in Notepad, copy all content, and paste it into your next reply.

I have a feeling its your power supply giving you the issue. I can't even find any amperage specifications on this model.
