problem disk

whenever i insert a floppy disk on my drive it says "disk in drive a is not formatted. then when i tried to format, it says windows cannot format it. why is that?
 
Could be a bad drive or bad disk. I had a FD drive that I hadn't used in years and that's what happened; the head reader could also be covered in dust/dirt from sitting there unused. Poor, poor old floppies...
 
Oh and it's been so long since I used them, I completely forgot! There are single density and double density disks and drives. Perhaps you are using a double-D disk in a single-D drive? A single density disk should work in a double density drive, however.
