problem with graphics card

I've just upgraded to Win 7 home premium ( from vista ) and got a new graphic card ( ATI radeon HD 5400 ) . I installed the CD that came along with the graphic card and updated it. After doing so , when I play games it lags and sometimes my screen will freeze and go black for a few seconds. It's starting to get really frustrating and I dont know what's the problem and how to go about it?

HELP!!

Install the latest drivers from ATI. The ones on that disc are probably far outdated.

what games are you playing, the 5400 series isn't made for gaming, more htpc style, you have to go low res and low shading on it, but also, what is it that you put it in? if you put it in a prebuilt it does need a new psu, oem psus arent worth a penny, and also what is your cpu, might not be good enough for the game.

OS : Windows 7 Home premium 64bit
AMD Athlon 64x2 Dual Core precessor
graphic card :ATI Radeon HD 5450 2GB
Ram : 4 GB
Motherboard : Can't find out what motherboard i'm having :S
PSU model : PT - 380ATX
Input : 220V , 50Hz , 6A
Output : +12V = 16A ( Yellow )+5V=18A( Red ) PG ( Grey )
-12V = 0.5A ( blue ) + 5Vsb=2.0A (purple ) +3.3V=16A( Orange )

I'm currently playing crysis 2 . I like to play first person shooter games.
My comp was custom built by a friend

that gpu is not made for gaming what so ever, also the psu might be a little underpowered, think it's a 4 or 450w gpu, first thing i'd do is lower the settings as much as possible, if that doesn't help then overclock a little and try to find the gpu controller sofware where you can fine tune some of the features, and lower them as much as possible.

I'm not very proficient in this area .. so i'm not sure how to go about lowering the settings and all or overclock :S
