Shared internet

I need to set up a server in an office that has 1 internet connection. Is it enough to set up security on my router and server app (database server), or should I be looking to install a dedicated line? This is for healthcare, so totally privacy is desirable.

What kind of router are we talking about, a consumer grade Linksys or business class router? I would look into a dedicated firewall before looking at a dedicated internet connection.
