Trying to remove a virus

I am using a Dell XPS 410 desktop, with Windows XP. It has recently become infected with a virus or Trojan, “c:\WINDOWS\systems32\kwinkmdt.exWin32:Downloader-1B[Trj]. It was discovered by Avast 4.7 Home Edition. I am notified by Avast at each start-up. Any recommendations on how to permanently remove this problem?
 
try THIS if avast can't remove it.
can't avast even quarantine it ?
you can also open MSconfig and disabling it at startup
start-->run-->msconfig-->"startup tab"

Let's see what's going on. Please download the HijackThis installer from http://www.trendsecure.com/portal/en-US/threat_analytics/HJTInstall.exe.

Run the installer and choose Install, indicating that you accept the licence agreement. The installer will place an shortcut on your desktop and launch HijackThis.

Click Do a system scan and save a logfile

When the Notepad window opens choose Edit -> Select All to select the entire log, and copy and paste the log into a reply post.
Most of what it lists will be harmless or even essential, don't fix anything yet.

1. Please download this file - Combofix to your desktop
2. Double click combofix.exe & follow the prompts.
3. When finished, it shall produce a log for you. Post that log in your next reply together with a new HijackThis log.

Note:
Do not mouseclick combofix's window whilst it's running. That may cause it to stall
