That would be the Royale theme for windowblinds, a quick search on wincustomize will find it for you.

*EDIT* Link to royale theme = http://www.wincustomize.com/download.aspx?LibID=1&SkinID=4654
Download it and then select to have the "thin start bar" in windowblinds.
