unable to format pen drive

Hi experts,
I have a pen drive and unable to format it.
Let me explain from the beginning.
I was making my pen drive booting from command prompt.
when i triggered CLEAN statement in command prompt my computer got restart.
when I replugged my pen drive. I am unable to format it.
I wend to disk management there it shows read only device and the partition is converted into raw partition. I tried to convert read only mode to write mode. for that used commands.

1. list disk.
2. detail disk
3. list volume
4. select volume
5. detail volume
there is was read only : Yes.
I converted yes to no.

Still it does not work.

Then I made changes in storage policy registry and still my pen drive is not working.

Please tell me the way to make my pen drive working.

Use diskpart and run the clean command on it again. Then try and format it.
