Word "Patially Read Only" files?

I have a fax cover sheet that is sitting out on the public server, and I want the users to only be able to fill in the blanks, so to speak, where it says TO:, From:, Phone Number:, etc. That way the fax form cannot be changed (except by admins) but they can type in the info that they need to put on there.

Any ideas?

Yes, that's the idea....but with text fields in the appropriate spots.
